Surgical treatment of Ectropion

Modified Kuhnt-Szymanowski technique for severe ectropion

a. Initial incision through skin and orbicularis m., 3 mm from and parallel to eyelid margin, extending 1 cm beyond lateral canthus. Second incision extends downward 1 to 5 cm.
b. Skin flap is undermined including orbicularis muscle.
c. A triangle of tissue is removed from the lid margin and conjunctiva equal to the excess amount of eyelid.
d. The conjunctiva is closed with absorbable suture using a continuous pattern. The eyelid margin is closed with a figure-eight suture.
e. Skin and muscle flap is pulled dorsoventrally and excess tissue excised in a triangle equal to the eyelid and tarsal conjunctiva.
f. Final closure with interrupted suture.
V-Y procedure for cicatricial ectropion

a. Measurements used to determine location of incisions.
b. V-shaped incision.
c. Flap freed so it can retract upward, allowing edge of eyelid to roll back.
d. Subcutaneous scar tissue excised.
e. Y-closure to midpoint.
f. Y-closure completed.
